To Print this regristration/Order Form:

1.  Right click on the form and select "Open image in New TAB" 

2. Open the New Tab, Right click off to the right side of the Form and select PRINT.  Follow the print prompts. (If your printed form/text is very small, before pressing the PRINT button, you may be able to select "More Settings", then change the "Scale" from 100% to 150%, and then click the PRINT button.

If you can not print it, just write on a sheet of paper the parts that you need to fill in and mail or deliver it in accordance with the instructions in Paragraph 3 below.
